What to expect during your initial visit with EVP:
Your initial appointment with EVP will begin with an examination (single area or entire mouth), we will discuss your diagnosis and treatment options.
What to bring to your first appointment:
- New Patient forms completed
- List of medications
- Medical and Dental insurance card
- Drivers License/ID
- Referral form and any x-rays if applicable
